We warmly welcome you to the Elders & Home Care page. Our aim is to support healthier, happy, stronger lifestyles for our Elderly & Ageing community. We are ready to help you with services tailored to your individual needs, with friendly support staff and social programs designed to support and connect you with the community. We understand how important it is for everyone to feel safe, strong in culture, independent and healthy.
This page is about supporting your independence and supporting people with disabilities and their carers. We are pleased to offer these services to Aboriginal people 50 years of age and over, people with moderate disabilities including younger people and their unpaid carer.

Home and Community Care Program for Younger People (HACC/PYP)
Doing What is Needed
Are you under 50 years of age and in need of support to manage the activities of daily living or caring for someone who needs support?
The HACC-PYP delivers a range of activities that may assist you to remain as independent and healthy as possible in the community.
The activities provided through HACC-PYP are:

In home services – supports clients to manage the activities of daily living and to maintain their independence. Supports include:
-
domestic assistance (cleaning)
-
property maintenance such as repairs and gardening
-
minor home modifications to assist people managing mobility issues to move safely about the house
-

Social support – includes planned activity groups and volunteer programs. Social support groups support people’s ability to remain living in the community by providing a range of enjoyable and meaningful activities. These activities:

support social inclusion
-
community participation
-
build capacity in skills of daily living.

While HACC-PYP continues to be funded by the state government, some existing clients will transition to the NDIS.
Commonwealth Home Support Program (CHSP)
Making A Difference
Are you over 50 and in need of support to manage the activities of daily living or caring for someone who needs support?
The CHSP is an entry-level home support program that helps older people to live independently in their homes and communities.
The activities provided through CHSP are:

In home services – supports clients to manage the activities of daily living and to maintain their independence. Supports include:

domestic assistance (cleaning)

property maintenance such as repairs and gardening

minor home modifications to assist people managing mobility issues to move safely about the house

The Elders Support Group – is about staying connected and creating a space specifically for our Elders and Ageing community members. Members:

plan outings together
-
enjoy basket weaving and other culturally based crafts
-
enjoy regular lunches
-
gather for a yarn

The friendly team at Gunditjmara can assist you with applying for CHSP assistance and assessment.
National Disability Insurance Scheme (NDIS)
Gunditjmara can provide NDIS services
Supporting those with special needs is an essential service to our community. Our friendly support staff offers a range of services that offer caring support and resources to maintain your independence and quality of care.
We provide culturally sensitive and appropriate services that assist with social and community participation. These include:

visits on country

shopping
Assisting with daily personal activities such as:

home cleaning
-
garden maintenance
We also assist with therapeutic support including:

Psychology – our Psychologist provides individual therapy and support to adult participants, including psychological assessment, report writing and recommendations. They assist participants to better understand the diagnosis and develop greater insight, as well as providing psychoeducation and support with communication, emotional regulation and self-esteem.
