What is the Home and Community Care Service?

The HACC Team: Bill Frances Glenda Michelle & Kenneth (KB)Gunditjmara Home and Community Care Service (HACC) has been designed to meet the needs of the frail aged/disabled and carers of Aboriginal Elders. A range of programs is offered to people who are unable to undertake various home duties due to frailty, disability or ill health.

The Service's aim is to reduce the premature placement of Elders into residential care accommodation, placing an emphasis on the continuation of independent living. Gunditjmara Home and Community Care Service is funded by the Department of Human Services. Programs are coordinated from the Gunditjmara Aboriginal Cooperative.

Eligibility is determined by assessment, and all applications are strictly confidential.

Before you receive Health and Community Care Services you will be visited by the coordinator of the program. The Coordinator will discuss your needs with you and help you access the appropriate services

Home Care Service

Designed to support individuals in maintaining an adequate woorn (house) in which to live. A range of domestic duties is carried out by the Home Care Worker. These include house-keeping, laundry, washing, shopping and mootpa (cooking).

Home Maintenance

This service is available to individuals who cannot cope with gardening duties or minor repairs. Workers carry out a range of tasks such as koorrookeet (cutting grass), edging gardens, fixing handrails and smoke alarms.

Day Activities

To strengthen social connectedness, individuals are invited to participate in a range of day activities. Activities include Elders Luncheons, Barbeques,  Picnics and Cultural events.

Respite

This service is designed to allow carers to have time away for a keenta (break) or to participate in family, social or community activities.

This service also provides time to go shopping or to appointments.

Adult Day Centre

Provides social and recreational activities for frail aged Aboriginal Elders and those with disabilities. Activities provide respite for families or carers.

Personal Care

This service provides assistance to those who need help maintaining personal hygiene. This includes assistance with showering, dressing, toileting, grooming and thakee (eating).

Client Transport

Transport is available throughout the Gunditjmara area. Eligible individuals fitting the criteria are picked up for appointments and social activities. Approval for transport will be given when the appropriate criteria are met.

Meals on Wheel

Home delivery of a variety of meals catering for special dietary requirements. Conveniently delivered to woorn.
